Simply create an account for free and gain access to the main dashboard where you can create your own waivers and questionnaires. Send out the link or QR code of the waiver or questionnaire to your clients and collect signed and completed documents in no time. Yes, absolutely! LenzVU is currently integrated with platforms such as MindBody Online & Mariana Tek, which allows you to have access to a variety of additional features.

You can have as many waivers, questionnaires or in-take forms as needed with our services (including basic). Our paid services charges are for storage of the waivers. Our basic package includes all the features and there are no limits to how many waivers you create. We include 3 months free storage for the waivers (calculated from the time the liability waiver was signed) and you can sign on for our other packages to store it for a longer period of time. You also have the capability to download the signed liability waivers.

There are no limits on the number of waivers that can be signed.
