LenzVU offers a simple, easy to use, and intuitive form designer that allows you to drag and drop elements to build your forms. Here is a quick guide for support on how you can get started.
You can have as many waivers, questionnaires or in-take forms as needed with our services (including basic). Our paid services charges are for storage of the waivers. Our basic package includes all the features and there are no limits to how many waivers you create. We include 3 months free storage for the waivers (calculated from the time the liability waiver was signed) and you can sign on for our other packages to store it for a longer period of time. You also have the capability to download the signed liability waivers.
Every form has a direct link (QR code). You can send out the link in an email to have completed ahead of time, or have the QR code posted in your facility for clients to scan from their own device. You can also have the form set up on an ipad from which clients can sign upon arrival.
There are no limits on the number of waivers that can be signed.
Yes, there is a way for you to see whether or not a client has completed a waiver. You will see an exclamation point on the class roster which indicates when a user has an unsigned waiver.
This is due to permission settings on your Mariana Tek account. Please reach out to the Mariana Tek Support Team and ask them to update the permissions for Admin users to be able to “edit tags”.
You may locate the most recently completed waivers on you LenzVU dashboard once you login. In order to see a complete list of completed, log into your LenzVU account. Using the toolbar on the left side, click “Reports”, and from the dropdown menu, click “Completed Waivers”. Here you will find all details related to the form submission, including which waiver version was signed, as well as the appropriate client information.
